Lots of changes, but the two big ones are CSV format output using
the new command line option --format, and consolidation of reads of
adjacent registers into larger blocks for better performance.

We have also added register definitions for Phoenix Contact EEM-MA600
power meters, and the ability to specify numeric register numbers on
the command line, which is useful when exploring and writing regtab
files for new devices.  And of course several other minor changes.
