Subject: [PATCH] Update dependencies and URLs in README file.

The old nsc-puppet-utils module has been split into multiple modules.
The one we use here is the concat module.  Update README file to track
this change.

We are also moving the official Git repository for this module to the
Linköping University GitLab server.
